Phone Pre-Setup
Install Drivers for Samsung Galaxy S3
Ensure previously activated (used) Verizon 4G LTE SIM is inserted into device during flash. Phone may be flashed without a SIM card, although data will work, data signal may not be seen and phone will show a notification that a SIM should be inserted.
Android 4.1.2
       Open Dial Pad
Dial *#22745927
At Hidden Menu Screen Select Hidden Menu
Select Enabled and Press OK
Next, press back to bring up the dial pad.
Dial *#7284#
Select Qualcomm USB Settings
Select DM + MODEM + ADB or RNDIS + DM + MODEM
Press Back Till Phone Is At Home Screen
Phone Is Ready To Be Provisioned
(Remember after flashing to return and put phone back to MTP+ADB)

Android 4.3
Open the dial pad
Dial ##366633#
Enable DM Mode
    If Houdini says the device cannot be provisioned but the device is showing in your device manager, please follow the these instructions.
     

    4.1.2

    1. With the device plugged:
    2. Dial *#7284# or ##3424# 
    3. Set UART to PDA
    4. Select Qualcomm USB Settings
    5. Set to MTP + ADB
    6. Disable USB Debugging Mode (Menu> Settings> Developer Options)
      1. If you don't see Developer Options, Enable them by going to Menu> Settings> About Device and Tap "Build Number" 7 times. This will enable Developer Options.
    7. Allow Drivers to load completely.  This can take up to 5 minutes.
    8. Device should now appear in device manager as Samsung Mobile USB CDC Composite Device
    9. Now..With the phone still plugged in, turn off and turn on (Restart the device)
    10. After the phone finished restarting, Dial *#7284# or ##3424# 
    11. Make sure PDA is still selected and select Qualcomm USB Settings
    12. Select DM+Modem+ADB
    13. Again allow the drivers to load.
    14. Detect

    1. With the device plugged in:
    2. Dial ##366633#
    3. Disable DM Mode
    4. Disable USB Debugging Mode (Menu> Settings> Developer Options)

    9. Enable DM Mode
    10. Again allow the drivers to load.
    11. Detect
      Post-Provisioning
      • ONLY PROCEED IF DATA IS NOT FUNCTIONING AFTER 15 MINITES.
        THIS MAY RESULT IN A LOSS OF DATA IF YOU CONTINUE AND DATA IS ALREADY FUNCTIONING.
        Bring up your dial pad
        Enter *#22745927 on the dial pad
        Click on the Hidden Menu Option to Enable the hidden menu (It will reset itself to disabled each time the phone power cycles)
        Enter *#197328640# on the dial pad
        At the Service Menu, follow the options below
        2-CDMA
        1-Common
        9-NextPage
        9-NextPage
        8-PREF MODE SET
        1-CDMA_HDR_LTE_Only
        Menu Button
        Back
        Reboot Phone
        2-CDMA
        4-Control
        9-CDMA BAND SELECT
        4-Automatic
      Programming
      • Open Dial Pad
        Dial *#22745927
        At Hidden Menu Screen Select Hidden Menu
        Select Enabled and Press OK
        Dial **772
        Select Edit
        Select MDN
        Enter new MDN (10 digit phone number) and press OK
        Select MSID
        Enter new MIN (10 digit number) and press OK
        Press Back then Done
        Phone will power cycle and programming is now complete

        • If the device is not functioning properly after a flash has completed, attempt to active with the Decimal form of the MEID (ex. 256XXXXXXXXXXXXXXX) instead of the HEX "IMEI" (ex. 990000XXXXXXXX)
